Connecticut Children's: Leaders in Advanced Solutions for Pediatric Co-Management

Providing the very best care for children and families is a result of collaboration between Connecticut Children’s medical staff and community providers. By working together to establish best practices and protocols for jointly managing patient cases, we have taken the lead in improving the quality of service received by all pediatric patients, whether seeing their primary care pediatrician or visiting Connecticut Children's.

Our focus on collaborative care has led to the successful development of referral guidelines, which outline when, how and under what conditions community providers should refer patients to Connecticut Children’s Medical Center. The guidelines are exemplary of our commitment to working with area physicians to establish consistent, straightforward steps for referral. In addition, the collaborative development of several Co-Management Programs has driven vast improvement in the quality of service patients receive. These programs expand the capabilities of community providers to provide appropriate levels of treatment for a number of conditions, bringing competent care closer to where patients and families live.
