Co-Management is a proven method for providing collaborative care to children. In our Co-Management programs, Connecticut Children’s medical experts team up with pediatric primary care providers to execute treatment in a way that is most efficient for physicians, effective in providing comprehensive patient care, and convenient for families.
The Co-Management model helps primary care providers expand the core competencies necessary to provide appropriate levels of care for these conditions in the medical home environment.
Connecticut Children’s has developed several comprehensive Co-Management Programs that include a central algorithm, visit templates for primary care providers, and handouts for the patient and family that help facilitate the co-management of patients between area health care providers and our medical staff.
Connecticut Children’s currently offers Co-Management Programs to assist health care professionals in treating patients affected by:
- Fibromyalgia & Chronic Fatigue Syndrome
- Lyme Disease
- Obesity Co-Morbidities
- Pediatric Voiding Dysfunction
- Premature Adrenarche
Interested providers should first log in to complete the Continuing Medical Education (CME) workshops for Co-Management. In addition to CME credit, providers who complete the the online training module will receive a follow up email with a username/password to access the Co-Management materials for that condition.
