Shared Expectations for Communication
Connecticut Children’s is committed to facilitating clear, consistent, and timely communication during key care transitions. The following pathways describe the recommended communication sequence during specific clinical circumstances. Dissemination of these pathways enables a transparent and shared set of expectations around the timing, mode and content of communication.
Referral Process to ED in Collaboration with Subspecialists
This is a recommended communication algorithm that applies to the unique situation in which a patient is referred to the ED following a verbal communication between the patients’ primary care provider (PCP) and one or more Connecticut Children’s subspecialists.
At Time of Patient Admission, Surgery/Change in Status, or at Discharge
This is a recommended communication algorithm that applies to the following situations: unplanned admissions, a surgery or change in patient status during the hospital course, and at patient discharge.
