What to Expect

Upon receiving a referral from your pediatrician or specialist, our scheduling team will call you to schedule an initial pain management evaluation.  Most patients will be scheduled for a New Team visit, which is a 2-hour multidisciplinary appointment in which you will meet with a pain management physician or nurse practitioner, a pain psychologist, an occupational or physical therapist, and a nurse.  Some patients may only need an initial 1-hour New Patient visit in which you will meet with a pain management physician or nurse practitioner, and you may subsequently be referred to Pain Psychology and/or Occupational or Physical Therapy.  We will determine which initial visit type is most appropriate for you during our referral triage process.

Upon scheduling your appointment, we will encourage you to sign up for MyChart, which is required should you chose to schedule any telehealth follow-up visits in the future.  We encourage use of MyChart to echeck-in for your visits, complete important pre-visit questionnaires, and communicate with your care team.  Please visit our MyChart page to learn more.

Your Preparation Checklist

  • Complete eCheck-In in MyChart up to three (3) days prior to your visit
    • Review/update your demographic and insurance information
    • Scan your photo ID and insurance card(s)
    • Sign consent(s)
    • Pay your copay, if applicable
    • Complete your intake Questionnaire(s)
  • It is important to complete your intake Questionnaire(s) in order to give your provider additional background information about your pain experience to better curate your pain treatment plan.  Please complete your Questionnaire(s) in advance of your visit via eCheck-In in your MyChart
  • Learn more about chronic pain and what to expect along this journey
  • The patient should dress in gym attire for the physical therapy portion of the visit
  • You may wish to pack water and a snack for this 2-hour visit
  • Please allow ample time for parking.  There is a cash-only parking garage attached to our Hartford office which charges $1 per hour.  After you park, go to Level B and turn right to enter the building.  Follow the tunnel to the end and take a right for the elevator or a left for the stairs.  Proceed to Level 5.  You will ring the doorbell at Suite 500, and a team member will assist you.  (Our Westport office has free parking located directly outside the building).
  • Bring a parent or legal guardian to your initial visit.  Patients should not come alone or with a non-parent or non-legal guardian.
  • Patients in DCF custody need to provide a 460R form to see “Pain Management” in advance of their appointment.  This can be emailed to PainManagement [at] connecticutchildrens.org (PainManagement[at]connecticutchildrens[dot]org) or faxed to 860-837-5209.
  • Please call us as soon as possible at 860.837.5207 if you wish to cancel or reschedule.  Please be aware that we do have a waitlist, and you may have to wait several weeks for a new appointment time.  We encourage you to plan accordingly to attend your visit.

Your Pain Management Journey

After your initial visit, your pain management provider will recommend additional follow up visits.  You may also be referred to follow congruently with a pain psychologist and/or Occupational or Physical Therapy.  You may schedule these visits each time you check-out from an appointment, or our scheduling team will give you a call afterwards.  Should you have any questions or need anything during your pain management journey, we are here to help Monday-Friday 8:30am-4:30pm at 860.837.5207 or by MyChart message.
