Connecticut Children’s Co-Management Program
Through our Co-Management program, Connecticut Children’s medical experts help pediatric primary care providers expand the care they deliver to patients by establishing guidelines that allow them to manage some complex conditions within their own practices.
Our referral guidelines outline when, how and under what conditions community providers should refer patients to Connecticut Children’s.
The guidelines help:
- Establish consistent, straightforward steps for referral.
- Drive measurable improvement in the quality of patient service.
- Expand the capabilities of community providers by making competent care more accessible to patients and families.
Connecticut Children’s has developed several comprehensive Co-Management modules that include a central algorithm, visit templates for primary care providers, and handouts for the patient and family that help facilitate the co-management of patients between area health care providers and our medical staff.
