Connecticut Children’s Vaccine Mandates For Staff Members

Connecticut Children’s is committed to doing everything possible to keep our team members and our patient families safe. That’s why we require our staff to submit proof of receiving an annual flu shot and all COVID-19 vaccine doses at the appropriate intervals, which includes booster doses. Booster administration is provided within these defined intervals depending on which vaccine you received:

  • 5 months from the second dose for Pfizer or Moderna recipients; and
  • 2 months from the first dose for Johnson & Johnson recipients

Note: If you were recently infected by COVID-19, you need to wait at least 30 days before receiving a booster shot.

Please refer to the Centers For Disease Control and Prevention’s website for guidance in choosing your booster shot.

Who The Mandate Applies To

The flu shot and COVID-19 vaccine/booster mandate applies to all staff of Connecticut Children’s, including team members, licensed independent practitioners, students, volunteers, contracted and traveling staff in all locations (including those who work remotely).

If you were exempt from our flu shot mandate in 2021 or the COVID-19 vaccine mandate for the initial series, please continue to follow the same guidelines. You do not need to apply for another exemption at this point in time.

COVID-19 Vaccine Booster Mandate Timing

Because our team members received the initial COVID-19 vaccine series at different times over the past year, Connecticut Children’s is not setting a specific deadline for everyone to submit their booster proof. Instead, we will continually track compliance and follow up as needed over time.

Where to Get the COVID-19 Vaccine Initial Series Or Booster Dose

Most of our team members were required to submit proof of receiving the full dosage of the Moderna, Pfizer or Johnson & Johnson vaccines in the fall of 2021. However, as new team members join our team or as team members come back from leaves of absence, they may still need to meet this requirement. 

If you need to receive an initial series of the COVID-19 vaccine or a booster dose, a clinic is available at Hartford Hospital at 85 Seymour Street from 9 a.m. to 5:30 p.m. ET, Mondays through Saturdays. Please make an appointment through Hartford HealthCare’s MyChart. Note that walk-ins are not available at this location – MyChart is the only way to schedule an appointment.

How to Submit Your COVID-19 Vaccine/Booster Proof

To submit your proof of receiving the COVID-19 vaccine initial series or booster dose, complete this form. If you need help please call the Help Desk at 860.545.8090. Please note, newly hired team members not yet eligible to receive the booster vaccine must also fill out the proof submission form and select the “new hire” option listed under the first question to ensure their compliance status is reflected accurately in the report.

Your COVID-19 Vaccine/Booster Proof Needs To Include This Information

Your COVID-19 Vaccination Record Card must provide a full history of your COVID-19 vaccination:

  • 2 doses of the Pfizer or Moderna vaccines plus one booster dose
  • One dose for Johnson & Johnson plus one booster dose

The card must also include your full name, date of birth, date, site and location of administration and the vaccine lot number.

Need Help Finding Your COVID-19 Vaccine/Booster Proof?

If you’re having trouble finding your COVID-19 vaccine or booster proof, these resources can help:

  • Access CT WiZ, the Connecticut Immunization Information System; or 
  • Print a vaccine record from MyChart.

If you received your booster outside of Connecticut, please provide a copy of your vaccine certificate from that state’s Immunization Information website.

How Do I Confirm My Booster Proof Was Properly Recorded?

You can confirm if your COVID-19 vaccine booster proof was properly received/recorded using this tool. Note that after submitting your proof, our Infection Prevention team will need to validate it. So, it could take 24 hours (or longer on weekends) for the proof to appear in the report. If after this time you are still listed as non-compliant, please reach out to your manager who can confirm with the Infection Prevention team.

If You Need To Submit FLU SHOT Proof, Please Note The Following

Our annual flu shot campaign for team members happens each fall. So, for the flu vaccine, at this time, you only need to take action if any of the following apply:

  • You weren’t able to submit your proof in the fall 2021 campaign due to a leave of absence;
  • You are a new team member; or
  • You previously received an approved exemption or deferment and it no longer applies

If any of these circumstances apply to you, you can receive your flu shot at your local doctor’s office, local pharmacy or big box store (e.g., Walgreen’s or CVS). Then submit your proof using this Form. If you are accessing this link inside Connecticut Children’s network, please click Company/Corporate Account” when logging in to use the form. Once your proof is submitted, it needs to be validated by our Infection Prevention team to ensure it includes all of the following details – please make sure your proof includes all of these. If this information is not included, you will be asked to resubmit your proof. It won’t be considered compliant until these details are received.

  • Name
  • Date of birth
  • Vaccine name/lot number, for both doses if appropriate
  • Date of vaccine, for both does if appropriate
