The Transitional Medical Care Program provides continuing multidisciplinary care for ongoing medical problems to high-risk infants after their discharge from the NICU.

In collaboration with subspecialists from Connecticut Children’s, our neonatal experts evaluate and manage complex medical issues including chronic lung disease. This experienced staff also monitors and manages weaning of infants off home apnea monitors, along with complete nutritional guidance for appropriate growth and development.

This program does not replace well-baby visits with a family’s primary care pediatrician or provide health maintenance services.
