Physician liaisons work to strengthen the relationship between Connecticut Children’s, our providers, and our community partners. We ensure our referring partners are fully educated with information about our world-class clinical programs, have knowledge of the new providers joining our team, and are informed of important hospital initiatives and expansion plans.

We foster an open line of communication with all partner practices and resolve any barriers or challenges they may face.

Physician Resources

Connecticut Children’s staff is here to help referring providers obtain medical records regarding their patients for whom we have provided care. Safeguarding patient information is our priority. We limit the faxing of medical records to urgent circumstances only, and in some cases, signed disclosure authorization is required before we can provide the requested medical information.

To request information regarding your patients for whom Connecticut Children’s has provided care, please contact the specialty directly. A disclosure authorization form is not necessary.

Patient or Family Requests

If your patients or their parents would like a personal copy of their medical records, they will need to complete and return our Authorization to Disclose Protected Health Information form. For more information on how to patients and families can submit medical records requests, please visit the Medical Record Information page.

Inquiries regarding requests for access or release of protected health information should be directed to the Health Information Management department by calling 860.837.5780.

Connecticut Children’s is committed to meeting the following standards regarding physician callback response:

  • Routine Callbacks: We aim to call back referring providers by 5:30 pm the same day the call is received.
  • Urgent Callbacks: We aim to call back referring providers within 15 minutes for urgent patient care issues.

We welcome your feedback. Please contact OneCall team at 1.833.PEDS.NOW (1.833.733.7669) to let us know how well we are meeting your physician callback needs.

The Referring Provider Advisory Board is a collaborative committee of community providers and Connecticut Children’s team members who meet quarterly to optimize the experience for a referring provider.

The Board works to identify key issues affecting a community based provider’s decision to refer or admit patients to Connecticut Children’s, and identify organization wide standards for access, communication, care coordination, co-management and customer service.
