The Care Network has contacts at all the Connecticut physician assistant programs. We’re pleased to announce the availability of virtual lactation consulting services through Nest Collaborative. They have set up a dedicated website and a form for providers and families to connect to their services. They also have examples of free materials you can order for your office. For insured families, services are available with no out-of-pocket cost (no co-pay, no deductible). Please contact them directly if interested.

To support patients and families who are part of our value-based contracts, the Care Network has nurse care coordinators who can help patients with complex conditions and those who utilize the ED and hospital frequently. They not only help to improve the quality of care for patients and families, but they also help to manage the total cost of care and expand the level of support you can provide to your patients.
