Annual Statewide Care Coordination Forum Connecticut Children’s Center for Care Coordination hosts an annual forum to convene professionals from all sectors who work with children and families to learn from experts and exchange ideas. The goal of each forum is to organize coordinators from all areas, including child health care, community-based agencies, education, behavioral health, and more in an effort to provide an effective and comprehensive care coordination system. 5th Annual Care Coordination Forum Connecticut Children’s Center for Care Coordination is pleased to host the 5th Annual Statewide Care Coordination Forum on June 22 from 8:00-2:30. The event is sponsored by the Connecticut Department of Public Health. It brings together nurses, social workers, community educators, physicians, advanced practice clinicians and other invested pediatric- and family-focused caretakers. The forum will host speakers from across the region who are experts in their field of pediatric care. The goals of the day are to increase awareness about challenges that are unique to families; create collaborations and partnerships that support family wellness; and improve access to patient care while addressing risk factors families face – all in an effort to build a comprehensive integrated system of care for children and their families.
